How To Order

Ordering Options Fax Services:
If you prefer to fax in your background request, you simply fax us the necessary information at 541-396-6860. We will fax you back a release form if required so please include your fax number. Once we receive the release form we will then process your background request.

Phone in Service:
If you prefer to phone-in your background requests, we will either fax or express mail you back the results. Remember you may be required to fill out a release form prior to a background being conducted. This is required by State and Federal law. CHAPPELLE ENTERPRISES operates in full compliance with Federal and State laws, and the FCRA. Release forms must be obtained in certain searches, CHAPPELLE ENTERPRISES provides those release forms.
